Activity Supervision & Verification of Hours -

  • Whenever possible, all CAS activities should be supervised by an adult. Ideally, this adult should not be related to you.
  • Before beginning a CAS Activity, give your potential supervisor a copy of the Supervisor Intro Letter
  • At the completion of every CAS Activity, you must have your supervisor complete the Activity Supervisor Evaluation Form.
  • If your activity is either (a) supervised by a parent or (b) not supervised by an adult - you must provide evidence/verification of participation and completion. Potential forms of alternative evidence include: picture/video, blogs, webpages, certificates of completion, awards, meeting notes and emails.

15th of the Month Deadline -

  • The Activity Completion Form, which includes the End of Activity Reflection, are due the 15th of each month for any activities completed in the previous month.
  • For example, reflections for activities that ended in September are due October 15. Failure to submit the documentation on time will result in the student not receiving credit for the activity.
